Nach dem Tod seines Idols Maximus (Russell Crowe) wurde Lucius (Paul Mescal), der Sohn von Lucilla (Connie Nielsen), außerhalb des Römischen Reichs in Sicherheit gebracht. Dem Einfluss römischer Korruption, Intrigen und Machtgerangel entzogen, wächst Lucius an der nordafrikanischen Küste in Frieden zu einem jungen Mann heran – bis er eines Tages von der Vergangenheit eingeholt wird. Seine neue numidische Heimat wird von römischen Legionen unter der Führung von General Marcus Acacius (Pedro Pascal) überfallen. Traumatisiert von tragischen Verlusten wird Lucius nach Rom verschleppt und an den Gladiatorenstall-Besitzer Macrinus (Denzel Washington) verkauft. Zur Unterhaltung des Volkes und der Co-Kaiser Geta (Joseph Quinn) und Caracalla (Fred Hechinger) soll er in der Arena um sein Leben kämpfen. Von unbändiger Wut angetrieben, will Lucius dem Imperium einen Schlag versetzen und vor allem Rache an dem Mann nehmen, den er als seinen ärgsten Feind ansieht: General Acacius, der allerdings mit Lucius Mutter Lucilla verheiratet ist.
